DeCare Dental claims can be made through the DeCare Pay & Claim system:

Pay & Claim

Step 1: At The Dentist

Following your treatment, you pay us. Ask us for a DeCare Claim form. We will fill in Section C. We will also give you a stamped itemised receipt which includes the detail requested in Section E.
If you require more extensive treatment, we can provide a detailed treatment plan. You can post this or email this to the DeCare customer service team (query@decaredental.ie) so that they can clarify exactly what is covered on the policy prior to treatment.
Step 2: Submit your claim

Complete sections A,B & D of the Claim form.
You can submit a claim as soon as a course of treatment is complete.
Submit the fully completed claim form along with the original receipts to:
